Salam,

I have a relative who has a strange problem. It started a few days a go.

When drinking water or juices or any liquids, once she swallows, some of the liquid moves towards her nose and a quantity actually gets up to her nose and almost gets out of her nostrils! :os She needs to tilt her head in a certain angle so that the water does not get up.

Her doctor is away now, and she does not want to see another doctor, but we need to know what is wrong.

I saw a previous CT scan for her head and it says that "left pyriform sinus in collapsed". What does that mean? Is it a good thing to have it collapsed or is it a bad thing.

She also had an MRI for the head (for another problem she had) and the report says "Bilateral maxillary sinusitis"

But these CT scan and MRI are several months old, and this problem is new.

What is this case? Could it be her sinuses or is it something else?

Thanks.

Ducklet Cat, how old is she? did she have surgery?

people who had previous surgery in ear, nose and throat can have that.

Maybe she has weak muscles in her mouth? or she has opening in her mouth?

Take her to see ENT doctor.

May God bless her with good health.

Thanks friends.
No she did not do a surgery, but she extracted a wisdom teeth a year a go.
She's almost 60.

What is the name of this case? I want to google more about it.

Thanks a lot.

Then maybe that could be the reason.

Ducklet, I'm sorry to hear about your friend's distressing condition

Pyriform sinus is a pouch on each side of the larynx below the base of the tongue

It is a disorder of this sinus that is causing the problems as it's on the route of swallowing

The MRI result of maxillary sinusitis has nothing to do with it, sinuses are like air filled bags in the face/skull, there are many pairs of them & they have a totally different function & are not related to swallowing or any disorders of swallowing

Is she a chronic smoker?

Any symptoms with solid food?

Thanks Evans.

Thanks Dr. Hitman.
No, she's not a smoker.

Below the base of the tongue?! :mmhmm:
Actually she had a mass in the root of the tongue, which was later diagnosed as "chronic abscess" after taking biopsies. She took antibiotics and anti-fungals for that. The abscess has been encapsulated now, and her doctor said no need to remove it.

Yes, she had problems with swallowing, and it was thought that the abscess was the reason. Swallowing is better now, but she still suffer with it, and under her tongue pains her and also her gums. The salivary glands are always red and pain her, but the doctor said they are not infected.
She also has a pain in her palate and keeps saying "kh, kh, kh" to scratch it because it itches and pains her.

The reason why she developed the abscess is not know. Could the collapse of
the Pyriform sinus be the reason for all that hassle? Actually I don't think her doctor saw the part of the reportof the CT scan that talk about the collapse of the pyriform sinus because he had biopsies reports that came in the same time, so he put more attention to then than the CT scan.

What does "collapsed" mean? Is it closed or has a whole or what? And what happens if that happens?
How do they fix that? If surgically, does it have to be full anaesthesia?

THANKS a lot. I really appreciate it.
